Research context

Semaglutide is one of the most widely studied GLP-1 receptor agonists. Compared with human GLP-1(7-37) it carries only two amino-acid substitutions, Aib at position 8 and Arg at position 34 (about 94% sequence identity), plus a C18 fatty diacid on lysine 26 that binds albumin. Aib8 blocks dipeptidyl peptidase-4 cleavage, and Arg34 replaces the second lysine of the native sequence so that Lys26 is the only acylation site. Together those changes turn a hormone that lasts minutes into a long-acting molecule.

Origin and molecular design. Semaglutide was developed at Novo Nordisk as a successor to liraglutide, and its discovery was published in the Journal of Medicinal Chemistry in 2015 (Lau et al.). The stated design goals were higher albumin affinity and full stability against metabolic degradation, and the fatty acid together with its linking chemistry, a C18 diacid joined to Lys26 through γ-glutamic acid and two OEG (AEEA) units, were the key variables. In that report semaglutide's GLP-1 receptor affinity was about three-fold lower than liraglutide's while its albumin affinity was higher, and its plasma half-life in mini-pigs was about 46 hours.

In research settings semaglutide is the reference GLP-1 receptor agonist: it is used to calibrate receptor-activation assays, as a positive control when screening new incretin analogs, and as a comparator for dual and triple agonists such as tirzepatide and retatrutide. Structural biology groups have solved cryo-EM structures of semaglutide bound to the GLP-1 receptor–Gs protein complex, which show peptide contacts similar to native GLP-1 but different motions within the receptor and the bound peptide (Zhang et al., Cell Reports, 2021).

Semaglutide is also a reference case in oral peptide-delivery research. Its tablet form is co-formulated with the absorption enhancer SNAC (sodium N-[8-(2-hydroxybenzoyl)aminocaprylate]); preclinical dog studies and clinical pharmacokinetic work localized absorption to the stomach, in a region close to the tablet surface, by a compound-specific transcellular route that requires SNAC and shows no evidence of an effect on tight junctions (Buckley et al., Science Translational Medicine, 2018).
